FDA Statement on Breast Implant Associated Squamous Cell Carcinoma (BIA-SCC)
The FDA recently released a Safety Communication concerning extremely rare reports of squamous cell carcinoma (SCC) and various lymphomas (not including BIA-ALCL) detected in the capsule surrounding breast implants. Based on the limited available information, these exceedingly infrequent cases occur around implants that have been in place for over 15 years. It’s important to note that the disease processes described in the FDA advisory are not unique to breast implants. Rare malignancies have been reported related to other types of implants, including dental and orthopedic implants. While the FDA’s role is to make the public aware of these reports, their current understanding is that these occurrences are extremely rare. If you have breast implants and are concerned about the FDA’s safety communication, here are current recommendations:
- Make sure to schedule routine follow-up consultations with your plastic surgeon regularly
- If you notice a change or abnormality in your breasts, such as a mass, swelling, or pain, report these issues immediately to your physician or surgeon
- Breast implant surveillance after surgery with imaging studies, including breast ultrasound and MRI, should be scheduled as per your physician’s recommendations

Despite the pandemic, patients spent over $9 billion on surgical and non-surgical procedures to improve their appearance. The top 2 surgical procedures were once again liposuction and breast augmentation surgery, although they swapped the lead from last year when breast implants came in first place. Body contouring with tummy tuck and breast lift surgery were also among the most popular aesthetic surgeries. Breast implant revision surgery with removal and replacement of implants rounded out the most sought after plastic surgery procedures. Of the non-surgical cosmetic procedures, the most popular remained unchanged: neurotoxins such as Botox, dermal fillers, and skin rejuvenation treatments.

On September 29, 2020, the FDA issued their latest guidance pertaining to Breast Implant Labeling, as well as recommendations for content and format of labeling information for both saline- and silicone gel-filled breast implants. These include:
- Boxed warning:
- potential risks of saline and silicone gel implants
- breast implants are not lifetime devices and complications may develop over time
- small risk of developing Breast Implant Associated-Anaplastic Large Cell Lymphoma (BIA-ALCL)
- Changes to screening protocol:
- silicone gel implants may rupture without causing symptoms or external signs – so called “silent rupture”
- in addition to MRI, ultrasound may be used as screening test to identify ruptured silicone implants
- the first screening should be performed 5-6 years after surgery, then every 2-3 years thereafter
- Patient decision checklist:
- patients should be provided with additional information regarding risks of breast augmentation surgery prior to undergoing the procedure
- Materials used in breast implants:
- composition of and materials used to create the shell and fill of breast implants
- chemicals released and heavy metals found in breast implants
Breast implants have been extensively studied and determined to be safe devices for use in patients. More recently, rare medical disease processes potentially associated with breast implants have been identified in a relatively small number of patients – namely Breast Implant Associated-Anaplastic Large Cell Lymphoma (BIA-ALCL) and Breast Implant Illness (BII). In light of these developments, the national plastic surgery societies are working with the FDA to develop standards and protocols that ensure maximal safety for all breast augmentation patients.

Breast Implant Recall: News, Facts, And Data About Textured Implants, The Allergan Recall, And BIA-ALCL
In the aftermath of the textured breast implant recall by Allergan on July 24th, 2019, the potential risks and adverse effects of breast implants have been the subject of many a news story and media attention. In order to help our patients understand the issues at hand, as well as their choices about textured breast implants, we have summarized salient data so they may make an informed decision. The association of a rare type of cancer, BIA-ALCL (Breast Implant Associated – Anaplastic Large Cell Lymphoma) with a specific type of breast implant, textured surface breast implants manufactured by Allergan), led to the recent recall of textured breast implants. Here are pertinent facts:
- The recall applies only to BIOCELL® textured surface implants made by Allergan. This includes saline and silicone gel filled implants with Allergan’s proprietary textured implant shell.
- Although BIOCELL® textured implants are no longer available for breast augmentation or reconstruction at this point, patients who already have these types of implants in place do not need to have any intervention, and the FDA is not currently recommending that they be removed. All patients with breast implants should be regularly monitored to evaluate implant status, and should undergo routine breast physical examinations.
- The risk of developing BIA-ALCL in Allergan textured implants is approximately 1:3800, whereas it is significantly less in other types of textured breast implants, and has not been documented around smooth shell implants.
- BIA-ALCL usually presents with findings such as late development of fluid around the implant, swelling of the breast, or a lump or mass in the breast. Definitive treatment involves removal of the breast implant and surrounding implant capsule, along with any abnormal tissue in the breast or capsule.
- Patients who have no symptoms related to their textured breast implants may yet wish to have them removed. Following implant removal, patients may opt for implant replacement with a different type of breast implant. For patients who do not wish to have their implants replaced, additional breast enhancement surgery with a breast lift may be indicated to produce the best aesthetic results.

FDA’s General And Plastic Surgery Devices Panel Meets To Discuss Breast Implant Safety
On March 25-26, the special FDA group that oversees breast implants, namely the General and Plastic Surgery Devices Panel, convened in order to hear from plastic surgeons, other physicians, and patients about the latest data and potential safety issues pertaining to breast implants. The use of implants for both breast augmentation and breast reconstruction was discussed, along with complications, breast implant revision surgery, and surgery for removal of implants (explantation). Different types of implants, including saline and silicone, smooth and textured surface, and round and anatomic variations, were discussed. Among patients who addressed the panel were those who had positive experiences from their breast implants, and conversely, those who had suffered adverse effects, such as the rare breast implant associated anaplastic large cell lymphoma (BIA-ALCL) and breast implant illness, an entity which has yet to be medically defined. Pros and cons, risk-benefit analyses, treatment recommendations, and suggestions for further study and regulations were presented.

In The News: Update On Breast Implant Associated Anaplastic Large Cell Lymphome (BIA-ALCL)
Breast implants are once again in the news, as the FDA just released an update on the exceedingly rare condition known as Breast Implant Associated Anaplastic Large Cell Lymphoma (BIA-ALCL). The 2 national organizations of plastic surgeons – American Society of Plastic Surgeons (ASPS) and the American Society for Aesthetic Plastic Surgery (ASAPS) – have also provided additional information regarding incidence, diagnosis, and treatment of BIA-ALCL. The following are some of the pertinent findings:
- Incidence of BIA-ALCL remains extremely low, with a lifetime risk of 1:3700 to 1:30,000
- BIA-ALCL occurs most frequently in patients with textured breast implants, rather than smooth walled implants
- BIA-ALCL usually presents as a mass or fluid collection around the breast implant
- Treatment of BIA-ALCL includes removal of the breast implant and abnormal tissue surrounding the implants (in the capsule)

FDA Releases Update On Breast Implant-Associated Anaplastic Large Cell Lymphoma (BIA-ALCL)
Breast implant-associated anaplastic large cell lymphoma (BIA-ALCL), is an extremely rare type of lymphoma that may occur in an exceedingly small number of women with breast implants. The FDA recently released an update to their statistics, with a total of 414 cases of BIA-ALCL reported to date. The vast majority of cases is seen in women with textured breast implants, and primarily silicone gel filled implants. Current literature puts the risk of the disease in women with textured breast implants in the range of 1:3,897 to 1:30,000. The disease is even more rare in women with smooth breast implants. BIA-ALCL is not a type of breast cancer, but rather a unique form of lymphoma specifically related to breast implants, and breast implants have previously been shown not to cause an increased risk of breast cancer. PROFILE (Patient Registry and Outcomes For breast Implants and anaplastic large cell Lymphoma etiology and Epidemiolgy) has been established as a collaborative effort with the Plastic Surgery Foundation and the American Society of Plastic Surgeons as a registry to report and track any cases of BIA-ALCL.
A very small number of women who have previously undergone breast augmentation may develop BIA-ALCL, which usually presents as swelling or a mass 2 or more years after their surgery. The disease is treatable and curable, with treatment often involving surgery with removal of the breast implant and the capsule surrounding the implant. All women with breast implants should continue to have routine follow ups with their plastic surgeon, and undergo breast examinations and radiologic screening (including mammogram, ultrasound, and MRI as indicated). Most women who have undergone breast augmentation have no complications, and of the few who need additional surgery the most common issues are related to implant rupture or malposition, or capsular contracture.

New Natrelle Inspira® Cohesive Silicone Gel Breast Implants Now FDA Approved
Another type of breast implant has received FDA clearance: Allergan’s Natrelle Inspira® style silicone breast implant, filled with the highly cohesive or “gummy bear” silicone gel material. This newest breast implant is available in addition to the previously approved Inspira® responsive silicone gel implants. It offers the same high gel fill ratio and contains stiffer, less malleable gel which provides greater shape retention and more upper pole fullness. Like other silicone implants, it is an option in cosmetic breast augmentation in women 22 years of age and older who wish breast enhancement surgery, or who seek implant exchange. Plastic Surgery Statistics For 2015 Show Increase In Procedures
The American Society for Aesthetic Plastic Surgery and the American Society of Plastic Surgeons revealed their national statistics for 2015, showing an overall increase in the number of procedures from the previous year. ASAPS reported significant increases in the following surgical procedures:
- Buttock lifts
- Fat grafting
- Liposuction
According to ASPS, the top 5 cosmetic surgical procedures last year were:
- Breast Augmentation
- Liposuction
- Rhinoplasty
- Eyelid Surgery
- Tummy Tuck
Botox and similar botulinum toxins, soft tissue fillers, and chemical peels led the cosmetic minimally invasive procedures. A significant increase in the number of men undergoing surgical correction of gynecomastia resulted in men accounting for 40% of breast reduction surgeries performed.

Tuesday evening marked the first Plastic Surgery program of 2016 on Sirius XM Doctor Radio, and Dr. Hutchinson was one of the guest plastic surgeons on the expert panel who answered patient inquiries about a host of cosmetic procedures. October Is National Breast Cancer Awareness Month
Breast cancer is the No. 2 most common cancer in women, affecting about 1 in 8 women in the US. Treatment options including surgery, such as total mastectomy, nipple sparing mastectomy, or lumpectomy with axillary node dissection, may be available to patients. In addition, breast reconstruction of the affected breast, as well as breast reduction, breast lift, and breast implants performed on the contralateral breast, may improve the cosmetic results following treatment.

New Silicone Breast Implants
Choosing the best breast implant. With the recent FDA approval of a new type of silicone gel breast implant, women interested in breast augmentation surgery have more choices than ever.
“Surgical techniques for breast augmentation and breast implants themselves are continuously being refined, increasing the safety and reliability of the procedure,” reports the American Society for Aesthetic Plastic Surgery (ASAPS). While there are two main types of implants – saline and silicone – there are considerations to be made for each one. For instance, there’s textured versus smooth implants and round versus shaped implants. Both devices may be implanted under the breast tissue or under the chest muscle
Saline implants consist of a solid silicone rubber outer shell filled with sterile salt water. This type of implant is usually inserted empty and filled at the time of surgery. One of the main benefits of this type of implant is that it can be inserted via a smaller incision than silicone implants. Although saline implants only require a small incision, they may not feel as natural as gel implants if the patient lacks sufficient breast tissue needed to cover the implant.
Silicone gel-filled breast implants have a silicone outer shell filled with silicone gel. Although silicone breast implants are pre-filled and may require a longer incision for implant placement, some people prefer them because they have a more natural feel. The newly approved “gummy bear” implants – also known as cohesive gel, highly cohesive gel or form-stable gel silicone breast implants – are a type of silicone implant. ABC News reports that the gummy bear nickname because when cut in half, the implant is stable and retains its shape, similar to gummy bear candy. These implants are available in round shapes as well as different types of teardrop shaped implants. They may be longer-lasting and maintain a more attractive and predictable shape. According to ImplantInfo.com, proponents of gummy bear implants say that other benefits associated with this type of implant include their ability to resist gravity, more infrequent hardening of scar tissue and the fact that they’re unlikely to leak.
One of the negatives associated with any type of silicone implant is that they may rupture without the individual knowing about it. Currently, the FDA recommends individuals with silicone implants get an MRI every other year starting with the third year after surgery to look for silent ruptures.

October Is National Breast Cancer Awareness Month
The American Cancer Society (ACS) has designated October as National Breast Cancer Awareness Month, and October 19th is National Mammography Day. ACS recommends yearly screening mammograms for all women in good health starting at age 40. In women who have a higher than 15% risk of breast cancer, breast MRI may be considered in addition to mammogram as part of yearly screening for breast cancer. Lifestyle modifcations such as limiting alcohol intake, maintaining a healthy weight and avoiding being overweight or obese, and engaging in regular physical activity and exercise have all been shown to lower breast cancer risk.
Patients who are considering plastic surgery of the breasts, including breast augmentation with saline or silicone implants, breast lift (mastopexy), or breast reduction surgery may be advised to undergo breast imaging prior to undergoing any aesthetic surgery.
New Breast Implants Approved By FDA
The FDA has approved a new type of silicone gel filled implant manufactured by Sientra, Inc. These implants, in addition to the other silicone breast implants available from Allergan and Mentor, will provide a 3rd option for patients over 22 years of age seeking breast augmentation for cosmetic reasons as well as for breast reconstruction patients of all ages.The approval is based on a 3 year study involving almost 1800 women, and is contingent upon additional follow up to evaluate the risks and complications of plastic surgery using gel filled silicone breast implants for both aesthetic and reconstructive purposes. Data obtained thus far supports the safety and effectiveness of silicone breast implants.
French Ministry Of Health Advisory Regarding Pip Breast Implants
The French Ministry of Health has recently advised the estimated 30,000 women in France who have PIP (Poly Implant Prothèse) breast implants to have them removed. These breast implants were not available in the US but American women who underwent breast augmentation abroad may have had them implanted. There is a reported twofold increase in the rate of rupture of the PIP breast implants, and health risks from ruptured or leaking implants are unclear.
